Transportation program that provides rides for people in and around their community

Additional Information

UCare Health Ride provides no-cost transportation to and from covered visits including:

• Medical
• Dental
• Mental health
• Substance use disorder
• Pharmacy
• Durable medical equipment

Rides may include a bus pass, taxi, or volunteer driver

Rides are limited to a maximum of 30 - 60 miles from home

Eligibility

Must be a UCare plan member including:

• Prepaid Medical Assistance Program (PMAP)
• Minnesota Senior Care Plus (MSC+)
• UCare Connect Special Needs BasicCare (SNBC)
• UCare Connect + Medicare
• UCare's Minnesota Senior Health Options (MSHO)
• Children on MinnesotaCare
• Adults on MinnesotaCare (limited to colonoscopy and mammogram services)

Application Instructions

• Bus passes may be given up to 14 days in advance
• Those who do not live on a bus line or cannot safely use public transportation need to call 2 days before an appointment to schedule a ride
