Family and Community Resource Navigator - Family Resource Center

Navigators within local community organizations help pregnant families and those with children find and connect with local early childhood and family well-being services.

Description

Navigators help families connect with the wide variety of services and supports listed below, make referrals on behalf of families when needed, and check-in periodically with families until navigation services are no longer needed. Many navigation agencies also offer some or all of the direct services listed below. Type of direct services will vary by location.

Navigators help families connect to local supports and services, including:

  • Pregnancy support services
  • Basic needs and economic assistance
  • Disability services
  • Healthy development and screening
  • Developmental and behavioral concerns
  • Family well-being and mental health
  • Early learning and child care, including preschool and early childhood family education
  • Dental care
  • Legal services

Navigation agencies may have staff that speak a variety of languages, that live in the local community, and represent similar cultural, racial and ethnic backgrounds of the families they serve.

  • Child and Teen Checkups (C&TC) Coordinators connect individuals to well-child care and screenings for eligible newborns, children and young adults
  • Family Home Visiting Expectant parents and parents or caregivers of young children can receive in-home support including education on pregnancy topics, parenting skills and connections to community resources.
  • Follow Along Program Program that helps to make sure children are learning and growing appropriately for their age
  • Parent Support Outreach Program (PSOP) A voluntary, supportive service that focuses on helping expecting and parenting families thrive.

  • WIC - Supplemental Nutrition Program A nutrition and breastfeeding program that helps young families eat well and be healthy. Learn about nutrition during pregnancy and feeding your baby and young children; meet breastfeeding goals; and buy healthy food using your WIC card.
