Coordinators connect individuals to well-child care and screenings for eligible newborns, children and young adults

Description

Preventive care keeps kids healthy! Coordinators at these agencies help individuals know if they are eligible for services and connect them with local clinics and agencies that provide comprehensive well-child exams. These exams include age-appropriate preventive, dental, mental health, and developmental screenings.

Eligibility

Newborns, children and adolescents through the age of 20 who are eligible for Medicaid

Fee

The cost for children without insurance is based on family size, income and assets.

There is no fee for children on Medical Assistance (MA) or MinnesotaCare (including through a health plan).

  • Child Foster Care (CFC) Licensing The local county office that oversees licenses for child foster care. Foster care is a temporary safe family home for children who have been placed by a court order or a voluntary placement agreement.
