Coordinators connect individuals to well-child care and screenings for eligible newborns, children and young adults

Description

Preventive care keeps kids healthy! Coordinators at these agencies are available to help individuals know if they are eligible for services and connect to local clinics and agencies that provide comprehensive well-child exams that include age-appropriate preventive, dental, mental health, and developmental screenings.

Additional Information

Well child health visits to monitor children's growth and development, screen for and identify potential concerns, and treat them to prevent bigger problems later on

The screening components are:

  • Vision and hearing check
  • Laboratory test, including lead, blood, and urine as needed
  • Immunizations, if needed
  • A complete head to toe physical assessment
  • Health education and counseling about health lifestyles
  • Nutrition assessment
  • Developmental assessment

Eligibility

Newborns, children and adolescents through the age of 20 who are eligible for Medicaid

ADDITIONAL INFORMATION: Any child or young adult, birth to 21 years of age

Fee

The cost is based on a sliding fee schedule for family size and income. Minnesota Medical Assistance and MinnesotaCare pay for the screening.
