Eligibility

To be served, the following must be provided:

• Proof of Address
• Picture ID for the adult picking up toys
• ID for each child ( 0-14 years only) in the household (example: medical card, birth certificate, school ID)

Application Instructions

Must apply at one of the S.N. Frazier District Churches.
Distribution date will be provided at the time of registration.
