Dental and outreach clinics provide dental services for people with developmental disabilities, Traumatic Brain Injury (TBI), and severe/persistent mental illnesses who are not able to get care from other community providers.

These are public clinics that are owned and staffed by the State of Minnesota.

Eligibility

Patients with developmental and intellectual disabilities, severe and persistent mental illnesses and traumatic brain injuries

Application Instructions

Staff will complete an in-take interview over the phone to make sure that new patients qualify for services.

New patients (or their guardians) will receive a packet in the mail with forms to complete and return. Once the clinic has received the paperwork, staff can schedule an appointment.

Fee

Most insurance plans are accepted. Patients enrolled in a Minnesota Health Care Program, including MinnesotaCare and medical assistance, are also eligible.
