Offers patient transportation for medical appointments, dental appointments and medication deliveries when all other means of transportation have been exhausted by the individual, family or friends.

Application Instructions

Patient should contact the Community Health Representative 24 hours in advance to schedule transportation after all other resources have been exhausted at (507) 430-5746

Medication deliveries are available after 2:00pm, Monday - Friday
