Transportation program that provides rides for people in and around their community

Additional Information

This program is offered through the Community Health Representative (CHR) program of the Health Division. CHR employees provide transportation to medical appointments.

The reimbursement program will pay for the most cost effective way to get to the closest provider with the level necessary care.

The Medical Volunteer Driver Program offers medical transportation by a registered volunteer driver who will transport you to the closest provider capable of providing the level of care you need.

Eligibility

Eligibility for the Medical Transportation program are Leech Lake Band members, Band members from other reservations, and the general public. Enrollment in a Minnesota Health Care Program may be required.
