Application Instructions

Preventative services: Monday - Thursday, 8:00am - 4:00pm
Restorative services: Tuesday - Thursday, 8:00am - 4:00pm

Clinic is located in the new Seminary Professional Building at Mayo Clinic Health System. Enter clinic through Door #1 at the end of Hill Street.

Fee

Accepts Medical Assistance and South Country Health Alliance. Charges a nominal fee for qualified uninsured patients.
