Services provided include:

• Primary medical care for preventative, acute and chronic issues
• Limited diagnostic services
• Mental health services and referrals
• Prescription medication
  • Pharmacy partnerships provides up to a 3 month supply of medications at no cost to the patient (does not include narcotics)
  • Assists patients in applying to pharmaceutical companies' Patient Assistance Programs to obtain higher priced medications at no cost

Eligibility

Must meet all of the following:

• Be uninsured
• Annual income at or below 275% of the federal poverty level
• Resident of Goodhue County or Lake City, MN

Application Instructions

Call the Care Clinic between 8:00pm - 4:00pm, Monday - Thursday.

Tuesday walk-in clinic: Registration is from 4:00pm – 6:30pm, and the clinic is open until 8:00pm. Services are provided on a first come, first serve basis.

Clinic is located in the new Seminary Professional Building at Mayo Clinic Health System. Enter clinic through Door #1 at the end of Hill Street.
