Description

Mental Health Case Managers coordinate services, design treatment plans and plan for crises to serve people of all ages with mental illness.

Plans for mental health services are customized for each person's needs and designed to help people live with as much independence within the community as possible.

Services may assist the person in gaining access to a variety of services including mental health, medical, social, educational, housing, vocational and recreational.

Additional Information

Case management services are available for eligible residents. A Diagnostic Assessment by a Mental Health Professional is needed to determine if a person is eligible for mental health case management.

Eligibility
